How to Get Started

The Cindy Haber Center coordinates services for adults and children with intellectual or developmental disabilities. Eligibility requirements defined by the Alabama Department of Mental Health and Alabama Department of Rehabilitation Services ensure available resources for those in need and may vary based on program. If you have questions about eligibility or want to apply for services, visit our Contact page and inquire via the form.

Who Can Make a Referral?

Referrals can be made by individuals, family members, or legal guardians.

How to Apply

  • Contact the Division of Developmental Disabilities Call Center at 1-800-361-4491.
  • Within two business days, an initial contact form will be shared with the Cindy Haber Center.
  • Once contacted by Cindy Haber Center, Inc., you will need to do the following:

    • Describe the needs and preferences of the person for whom services are being requested.
    • Provide the Cindy Haber Center staff with information on the person’s current situation, personal and family history, and the person’s Medicaid eligibility status.
  • Additional information will need to be completed by Cindy Haber Center, Inc.

Who is Eligible for ID (Intellectual Disability) and LAH (Live at Home) Services?

Eligibility is determined by:

  • A psychological evaluation with the IQ score below 70 and a review of all past intellectual assessments and IQ scores.
  • Documentation that the person in need of services has challenges with adaptive functioning in three or more of the following major life activities prior to the age of 18:

    • Self-care
    • Receptive and Expressive Language Development and Use
    • Learning
    • Self-direction
    • Mobility
    • Capacity of Independent Living
  • Currently the ICAP is used for documentation for the requirement above.
  • Documentation that the person’s level of adaptive and intellectual functioning occurred prior to the age of 18 (developmental history).
  • Documentation of family and person’s historical information.

Project Sunrise Eligibility

A child is eligible for Early Intervention services if he/she is under 36 months of age and has a 25 percent or greater delay in one or more of the five major areas of development: Physical (Motor), Social/Emotional, Adaptive (Self-Help), Cognitive or Communication.

A diagnosis with a high probability of developmental delay can also be used to determine eligibility.

Supported Employment Milestones Program of Baldwin County

The Supported Employment Milestones program provides initial and long-term support to help individuals with intellectual and developmental disabilities be successful in the workforce.
